Enable Survey: By activating the "enabled" checkbox only, your survey will be voluntary. Additionally activate the second checkbox to make it mandatory. If your users don't fill it out, they won't be able to send their photos.

Title & Info: If you leave these fields blank your users will only see the survey question and answer options.

Question and Answers: You can add unlimited answer options (one answer per line). Your users can only select one of it.
